Breast surgery is undertaken in two cases:

– Mammal augmentation

– Breast reconstruction

Mammal augmentation:-

Breast surgery for breast augmentation involves making a single incision in or near the breast and inserting the implant either between the breast and the chest muscle (subglandular placement) or behind the chest muscle (submuscular placement) .

A second incision can be made in the natural crease under the breast, called the inframammary crease approach.

A third approach is to make the incision at the junction of the colored part around the nipple (areola) and the natural skin, called the periareolar approach.

Breast reconstruction: –

One of the most valuable surgical procedures used today is breast reconstruction. This is a surgical procedure to restore the appearance of a breast in women who have had their breasts removed (mastectomy) to treat breast cancer. The surgery reconstructs the contour of the breast and, if desired, the nipple and the areola.

Although the reconstructed breast does not have a natural feel, the surgery can provide an excellent breast-like result.

There is a choice between immediate or delayed reconstruction after surgery.

– Immediate reconstruction or the one-step process involves placing a breast implant directly after the mastectomy. Few women are able to go through a one-step process – the placement of the implant at the time of the mastectomy. In autologous tissue reconstruction, the doctor transfers a section of skin, muscle, fat, and blood vessels from a part of the body to the breast to create a new breast mound. Skin and tissue must be augmented with a breast implant to achieve the desired breast size.

– Delayed reconstruction: It consists of the placement of a tissue expander which is eventually replaced by an implant.

Reconstructive breast surgery does not interfere with future treatments such as radiation therapy, chemotherapy, or detection of recurrent breast cancer. It also does not increase the risk of breast cancer recurrence.

Recovery time is usually six to eight weeks. Risks of bleeding or infection are possible. It can take up to 1-2 years for the tissues to fully heal and the scars to disappear, but the scars never completely disappear.

Lumpectomy –

Lumpectomy is a method of surgical treatment for women who have been diagnosed with breast cancer. This is breast-conserving surgery because the surgeon only removes the part of your breast that contains the tumor (the “lump”) and some of the normal marginal tissue around it. If cancer cells are found in the margins, further surgery (called re-excision) is needed. Most women receive five to seven weeks of radiation therapy after a lumpectomy to remove any remaining cancer cells.

The combination of lumpectomy and radiation therapy is called breast-sparing therapy.

Lumpectomy is a breakthrough surgery for early-stage breast cancer, affecting approximately 50% of women. The location, size, and type of tumor are important when considering breast cancer surgery options. Experts agree that since survival rates are equivalent to mastectomy, lumpectomy followed by radiation therapy is the preferred treatment for most women with early-stage cancer. For women who have undergone a lumpectomy with clear margins, followed by radiation therapy, the average risk of non-recurrence is between 85 and 90%.

Post-operative precautions:-

– Do not lift anything heavy. Do not do repetitive movements (such as ironing or vacuuming) with the affected side for at least 4 to 6 weeks.

– Do not swim or play sports until your scar has healed – approximately 4-6 weeks.

– Women should wear a well-fitting bra day and night for about a week after surgery.

– Protect your hand and arm from infections.

– Keep the fluids moving in your operated arm. Do not wear tight sleeves, bracelets, wrist watches or rings on this arm.

– Do exercises to stop swelling and improve circulation. For example, try squeezing a rubber ball with your hand for a few minutes a day.

Risks Associated With Lumpectomy:-

– There is usually a loss of sensation in the breast, depending on the size of the lump removed. Some or most of this ability to feel gradually returns.

– After surgery, the two breasts may not match in size and shape.

– Seroma – an accumulation of fluid under the arm occurring approximately five days after surgery. It is likely to go away in a few weeks or needs to be drained by a doctor.

Breast Surgery Risks –

All surgery carries some risk and it is important that the patient understands the underlying risks before making a decision about it.

Complications vary from person to person and each patient experiences their own individual risks, but the patient should always have a complete idea of ​​possible outcomes.

A. Capsular contracture – occurs if the scar or capsule around the implant begins to tighten. It can make the breast hard. Additional surgery may be needed to remove the scar tissue. In some cases, the breast implant may need to be replaced.
